At this time, we do not accept walk ins and we are only in the office if we have an in office visit scheduled. If you have any urgent needs please call use at (702) 997- 4455 and if you are having and emergency please call 911 or go to the closest ER.

We are located at 5580 West Flamingo Rd suite 107 Las Vegas NV 89103. Major cross streets are Flamingo and Lindell. Our suite is the last suite located on the right side of the building. There is plenty of available parking right outside our suite for our patients. If you have any issues finding us, give us a call at (702) 997 – 4455.

Our registration staff will ask you to fill out registration and health history forms. We ask that new patients arrive 30 mins before scheduled appointment time to complete your paperwork. If you would like to complete a majority of the forms online prior to your appointment  we can sent you a link for our patient portal or email them to you.

Please bring all medications you are currently taking and your insurance card or copy of your card. If your insurance requires a co-pay/deductible, please be prepared to pay that at the time of your visit. In addition, it is helpful to write down a list of questions for your provider so that we can ensure that we answer all of your questions in the exam room.

Yes, we take many insurances plans, commercial and government. Insurances we take can change quite frequently and if you would like to know if we take your insurance please call us at (702)997-4455 and we will be glad to answer any of your insurance questions to the best of our abilities.

Yes, we do accept certain types of Medicaid. For more information on using Medicaid, please call us at (702) 997-4455.

Balanced Life Behavioral Health knows the importance of your health and the added costs of being healthy, that’s why we offer competitive prices on all our services avoiding that your health becomes a financial problem. If you are struggling with mental health issues and need assistance, we do offer a sliding scale which requires some additional paperwork to be filled out.

Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan. We are only able to provide quotes for the office visit portion of your visit. If the services are not covered, we can work with you to develop a payment plan.

We have limited office hours currently due to COVID we are not accepting walk ins at this time. We are only in office when in person patients are scheduled. We do try to schedule patient as soon as possible and will work with you to find a time that works for you. Call us at (702) 997-4455

If you need to cancel an appointment, please call us at (702) 997-4455. Remember to cancel your apt as soon as possible and if you know you will not show. This allows us to maintain our commitment of not over booking appointments due to people not showing up to their scheduled patents.

Every patient in our practice has his or her own provider. Your provider is assigned by how soon you need to be seen and the insurance you have. If you need to make an urgent care appointment and your regular doctor is not available, we may ask if you would like to see another of our doctors or one of our nurse practitioners.

When you have an appointment scheduled with your own provider, that is who you will see. If you need to schedule an appointment urgently and your provider is not available, we may ask if you would like to be seen by another provider. This often makes it possible for you to be seen sooner.

“Evidence-based medicine” means medical care that is up-to-date with the most current recommendations. Medical research is constantly discovering new ways to identify, prevent, and treat illness. We pride ourselves on being aware of that research so we can provide our patients with the most effective medical care.

A nurse practitioner is a registered nurse (RN) who has completed additional nursing education and training in diagnosing and managing common as well as complex conditions. Because of their advanced training, Nurse Practitioners are fully qualified to perform many of the tasks of a physician, including the performance of physical examinations, treating illnesses and prescribing medications. Nurse practitioners have a particular focus on prevention, wellness and patient education.

We ask that you contact your pharmacy before your medication runs out to request a refill. Providers may have to review your medical record before prescribing or refilling any medications. Please note that any new medications may require a patient visit and all controlled substances always require a visit. Any new prescriptions or refills can take up to 72-hours to complete. Please always be sure to call your pharmacy before your medication runs out.

The Health portal enables you to communicate with our doctors, nurses and staff members easily, safely and securely from the comfort of your home. The Health Portal is used to request non-urgent medical advice, request appointments, and obtain visit summaries, order medication, view lab results and access patient education. At the time of your first visit, you will sign up for the Health Portal and be given instructions on how to log into the patient portal on our website.

Telemedicine services allow you to see a provider via a secure video on your phone or computer for your appointment. Telemedicine visits are a great way to have a visit with your provider without ever leaving the comfort of your home! This service is offered to ESTABLISHED patients that do not require a physical exam. Most insurance plans cover the video appointment.

Forms are dropped off at the front desk. Depending on the type of form, like FMLA, Disability and Life Insurance may take up to 5-10 days to be completed. Some paperwork might require an appointment with your provider for completion.
